Submit Documentation and Applications for Assistance
The LifeStyles of Maryland team is committed to working with those in need to help them connect with any available financial assistance that can help you along your road of Help, Hope and Transformation.
If you are in need of financial assistance, please FULLY complete and digitally sign the below form. Before beginning the form, please ensure that you have the following documents ready to upload:
- Driver’s license or government-issued identification card
- Social Security cards or names and SS numbers for everyone in your household
- Copy of your lease or mortgage statement
- Any At-Risk Documentation (for example, court or eviction notice, approval or welcome letter, or utility bill termination/disconnect notice)
- Proof of income for every household member for the last 30 days (or documentation showing termination or layoff)
Please note that applications cannot be reviewed if these documents are missing.
