Nutrition Assistance Outreach Request for Proposals

Apply for Outreach Funding

The LifeStyles’ Nutrition Assistance Outreach Program expands our SNAP-approved program. SNAP provides eligible low-income households with a monthly subsidy to help them buy the food they need. An application must be submitted for each household to determine eligibility. Community-based organizations and other entities, along with the local Departments of Social Services, help households apply for such benefits.

LifeStyles of Maryland Foundation, Inc. is one of the lead agency recipients for the MD Department of Human Services’ (DHS) SNAP Outreach grant contract for federal fiscal years 2025–2027 (October 2024 – September 2027). We are looking for eligible sub-recipient organizations to assist in reaching eligible individuals and families in targeted counties.

SNAP Outreach grant partners provide the following services:

Educate the community regarding the SNAP program and eligibility.

Conduct and participate in outreach activities.

Assist households in applying or recertifying for benefits, either through an electronic or paper application.

Help individuals apply for other public assistance benefits as appropriate.

Agencies who are SNAP Outreach recipients should provide staff to:

Attend and/or host community events that market SNAP benefits.

Train using the state’s electronic public assistance benefits system, MD THINK.

Attend required stakeholder meetings.

Help people apply for benefits, assist with scheduling interviews, and upload required information.

Provide technical assistance for applications, recertifications, fraud applications, and other public assistance enrollments.

TARGETED POPULATION

Funding should target low-income and/or hard-to-reach households residing in one or more of the following counties: Calvert, Charles, St. Mary’s, Prince George’s, Anne Arundel, and Montgomery Counties. Outreach funds should specifically focus on low-income households.
